A cancer therapy cleared Phase 3 yesterday that was made for exactly one person. Sequenced from her tumour, built for her immune system, never manufactured again. For a century medicine has meant a compound discovered once and given to everyone it fits. That model built most of what works, and it selected hard for diseases with enough patients to justify the discovery. Everything else waited. What read out yesterday is the first proof at scale that treatment can be a procedure rather than a product. Not a molecule you find, but a method you run, on one person, from their own biology. The consequence takes a while to land. If a therapy is made per patient, the size of a disease stops determining whether a therapy can exist. What determines it instead is whether anyone will pay for the run. That's a better problem than the one we had. It's also a decision rather than a limit, and decisions can be made differently. A hundred thousand people with the same rare mutation are not a market. They can still be a fund. The century of blockbusters is ending on its own.
